Global Insights on FHIR Adoption, Regulation, and Implementation

About the Report

Since 2023, HL7 International has published the State of FHIR Survey, capturing how HL7 FHIR is being applied worldwide. The 2025 survey, conducted in April, gathered 82 expert responses from 52 countries—more than double the reach of 2024. Respondents included HL7 affiliates and national standards leaders, providing a uniquely global view of FHIR adoption.

This year’s survey focuses on:

  • Adoption trends – shifts in current use and year-over-year changes
  • Regulation – mandates, deadlines, funds and enforcement
  • National standards – core FHIR guides, implementation patterns and use cases
  • Implementation realities – drivers, tools, exchange mechanisms and challenges

While results are primarily qualitative, they reveal how governments, organizations and implementers are navigating the opportunities and barriers of FHIR in 2025.

Key Findings

  • 78% reported regulation for electronic health data exchange
  • 73% said FHIR is either mandated or formally advised
  • About half reported adoption deadlines, and about half have funds available to support implementation
  • A strong majority (67 of 80) have a national organization for health data standards
  • Most respondents (65 of 82) indicated their country has a base/core FHIR implementation guide (at least under development)
